Before your appointment

Three things to know before you arrive.

01

Arrive with time to settle in

Please plan to arrive 15 minutes before your appointment for check-in.

02

Bring the essentials

Please bring a valid photo ID, your current insurance cards, and the completed new-patient paperwork.

03

Need another way to complete it?

Call the office to have the packet mailed, or arrive 30 minutes early to complete it before your visit.

New-patient paperwork

Download, print, complete, and bring it with you.

The current packet is ten pages. Completing it before you arrive gives the office more time to focus on check-in and your questions.

What to expect

A first visit built around understanding.

Knowing the shape of the visit can take some of the uncertainty out of walking through the door.

  1. 01

    Check in

Give yourself time for paperwork and administrative questions before the appointment begins.

  2. 02

    Share what is changing

You do not need the right medical term. Explain what hurts, when it started, and what it is keeping you from doing.

  3. 03

    Understand the next step

The visit is an opportunity to examine the concern, discuss what may be contributing, and talk through appropriate options.
